Rehabilitation Worker
East Bristol Intermediate Care Centre is a 17 bedded rehabilitation centre that provides a short term rehabilitation service to adults who are recovering from physical injury or trauma. The service user group changes frequently and our maximum stay is up to 42 days.

£21,074 - £23,111 pro rata plus shift payment.
30hr permanent post

4 week rota in place including:
Working alternate weekends
Shift work. Earliest shift begins at 7am with latest shift concluding at 10pm.
Some bank holidays.

Our main aim and purpose at EBICC is to enable Service users to maximise their independence and return to their own home safely.

The successful applicant's duties will include (please see Job Description for full duties):

To be a key-worker and carry out agreed rehabilitation plans under the guidance of the multi-disciplinary team and work with service users on an individual basis. 
Take a lead role in coordinating/assisting with Service User review meetings, ensuring all relevant parties are involved and their views considered and to co-work with other multi-disciplinary team members/other professionals to plan/coordinate timely discharges.
Document all observations and interventions, maintaining accurate records in line with legal requirements. To contribute to case records, in writing and electronically and contribute to the end of service report.
Assist in the control, recording and issuing of drugs and medicines as prescribed by the medical practitioner in accordance with directorate policies and instructions while facilitating self-medication as indicated by individual rehabilitation programmes.
